Check Claim Status – College of Charleston


If you are a student and would like to check on the status of a claim that you or a provider submitted to UnitedHealthcare, you will need to set up a My Account if you have not already done so.

Please visit our My Account center to log in to an existing account or to create a new one.

File an Appeal – College of Charleston


To file an appeal, please include the following information:

    1. 1. A letter requesting an appeal to your claim(s), including your:
      • Name,
      • Address,
      • Phone number,
      • UnitedHealthcare Student ID number,
      • Date of service for your injury/sickness,
      • Claim number(s) (located on the top of your Explanation of Benefits), and
      • Email address.

2.  A copy of your Explanation of Benefits for the claim(s) in question.

3. Medical Records including all test results from all providers visited for the specific injury/sickness that you are appealing.


Once we receive the documentation, your appeal will be reviewed and a written response will be mailed to you. The response will include what the findings were, if the appeal was approved or denied, and the reason for the final decision.

If you have a question about your claim denial, you may call our Customer Service Department at telephone number 1-888-251-6253 for further explanation to informally resolve your appeal. If you are not satisfied with our explanation of why the claim was denied, you or your authorized representative may request an internal review of the claim denial. Global Emergency Services (GES)

Waive Your School’s Insurance – College of Charleston


This page is for those students who wish to opt-out (waive) out of the coverage under their school’s student insurance plan. The school’s student insurance plan can only be waived if the student has coverage under another acceptable insurance plan.

The first step to waive out of your school’s student insurance coverage is to verify that you are an eligible student.

Are required to purchase this plan unless proof of comparable coverage is furnished.

Please Note:  If you waive the insurance you will not be eligible to enroll until the following school year unless you experience a Qualifying Life Event.

J-1 and F1 Student Waiver Instructions

  • Annual/Fall Waiver Deadline: 8/31/18
  • *Spring & Spring/Summer Waiver Deadline: 1/18/19

After you verify that you are required to waive and have other insurance you will need:

  • Your current health insurance plan ID card,
  • Your health insurance brochure or plan description in ENGLISH


Once you have completed the waiver form, an email will automatically be sent to the email address you provided notifying you of the waiver request decision. If you receive a:

  • Approval e-mail – Your waiver was approved. You should print this e-mail and save it for your records.
  • Denial e-mail – Your waiver was denied. If you would like us to look this over or if you answered a question incorrectly, please contact us.
  • Pending e-mail – Your waiver is now pending for review. You need to send a copy of your policy in ENGLISH to waiver@pghglobal.com or by fax to 267-406-4191.

 

OPEN ENROLLMENT PERIODS

If you have eligible Dependents in the Fall or are a student in the Fall semester and are eligible to purchase coverage and you choose not to enroll for coverage before the Annual/Fall Enrollment Deadline, your Dependents or you will not be eligible to enroll again until the start of the next fall unless they experience a Qualifying Life Event during the year

*Spring and Spring/Summer enrollment periods are for new incoming and renewing students and their dependents only.


ELI Student Waiver Instructions

  • Fall 1 Waiver Deadline: 8/31/18

 Qualifying Life Event

If you did not enroll during your open enrollment period and have since lost coverage under your original plan, you may qualify for a Qualifying Life Event upon providing proof of involuntary loss of coverage and payment within 30 days of losing coverage (example: marriage, divorce, loss of job, etc.)

The enrollment form, check or money order, and letter of creditable coverage must be received within 30 days of losing coverage. After 30 days the student will no longer be eligible to enroll in the plan until Fall of the following school year.

Permanent ID Card


In keeping with our Go Green initiative, permanent ID cards will be available by request only. Should you wish to obtain a permanent ID card, you can request one to be shipped to you by accessing your My Account. Permanent ID cards will be shipped  24 – 48 hours of your request.

Medical Bill Saver

What is Medical Bill Saver?
The Medical Bill Saver feature provides skilled negotiators who will work with your providers to lower your out-of-pocket costs on your medical or dental bills over $400 not covered by your insurance. We can attempt to negotiate bills to help reduce the balances that apply to deductibles and coinsurance.

How can it Help?

  • Negotiation can result in 25-50% savings.

Sports / Athletic Insurance – Westminster College


Welcome to the Sports Athletic Insurance Center! Follow the Instructions below to get started.

As soon as the injury occurs, contact the Athletic Director, Athletic Trainer or a designated person in the Athletic Department.


They need to document the injury and notify us with the injury details. This is one step towards a speedy turn-around time on your claim.

What does my provider need to know? 

Your provider will need to know where to send your claim, the phone number to call for benefits and to verify coverage, how they can submit your claim and what they should include. The Claim Submission Information Form will answer these questions. Please make sure to take this form with you when visiting your provider, as it supplies your provider with accurate instructions on how to submit the claim on your behalf.

Can Someone Help Me?

If you would like someone from your Athletic Department or a parent to assist you with filing your claim and to be able to discuss details of your claim with our claim department, you will need to complete and sign a PRA (Personal Representative Appointment) Form. The Yearly Authorization Form gives us permission to discuss any and all medical conditions with your Personal Representative, throughout the school year. This form is filled out once and is good for every injury for the entire school year.

Submit a Claim by Mail


It’s easy! Just download the claim form below. Fill out the first page with your personal information, dates of injury or sickness, the cause of the injury or date of first symptoms for a sickness, sign your name with today’s date, and attach all receipts. Please fill out the entire first page and be sure not to leave anything blank.


Your doctor will need to fill out the second page of the form.

Submit a Claim Electronically

Your Doctor can also submit a claim electronically using Emedeon (formerly WebMD). This gives a faster turn-around time than submitting a claim by mail. Check with your doctor to see if they are a participant. If they are, show them your ID Card. The Emedeon information is printed on the back of every ID Card.
