Hard Waiver Student Eligibility

All full-time domestic Undergraduate students taking 3 Units (12 or more credit hours) and all full-time domestic Graduate students taking 9 or more credit hours, are automatically enrolled in this insurance Plan at registration, unless proof of comparable coverage is furnished.

All undergraduate international students, all graduate international students, exchange students, Study abroad students, ESL (English as a Second Language) students and visiting scholars, regardless of credit hours are automatically enrolled in this insurance Plan at registration, unless proof of comparable coverage is furnished.

  • Annual Enrollment Deadline: 9/9/16
  • Spring/Summer Enrollment Deadline: 2/9/17 (For new incoming students only)

All Full Time Domestic Undergrad and Full Time Domestic Graduate Students

All eligible full-time students will be automatically charged for the Student Injury and Sickness Insurance Plan. If you fail to complete an online waiver by the published deadline you will be automatically enrolled.

Dependent Enrollment Instructions

Your school does not offer dependent coverage on your plan.


Open Enrollment Periods

Open Enrollment Periods for all students: If you are a student in the Fall semester and eligible to purchase coverage and you choose not to enroll for coverage before the Annual Enrollment Deadline of *September 14, 2016, you will not be eligible to enroll again until the start of next Fall unless you experience a “Qualifying Life Event” during the year.

*For new students in the Spring semester, your open enrollment deadline is February 2, 2017.


Qualifying Life Event

Students who did not enroll themselves during the open enrollment period may be able to enroll in the policy if they experience a Qualifying Life Event.

Qualifying Life Events include:

  • Acquiring a new family member
  • Marriage/Civil Union
  • Divorce
  • Loss or change of Job
  • Expiration of Benefits

If you meet the requirements for a Qualifying Life Event, you will need to provide the following:

  • Proof of qualifying life event/proof of involuntary loss of coverage
  • Enrollment Form
  • Payment

All documents and payment must be received within 30 days of the qualifying life event. After 30 days, the student will no longer be eligible to enroll in the plan, until fall of the following school year.
