File a Medical Claim

Welcome to the File a Medical Claim Center! We understand the hassles that come along with filing your claims, which is why we want to make this process as quick and painless for you as possible. Below you will find all the information you will need to file a medical claim.

Submit a Claim by Mail

It’s easy! Just download the claim form below. Fill out the first page with your personal information, dates of injury or sickness, the cause of the injury or date of first symptoms for a sickness, sign your name with today’s date, and attach all receipts. Please fill out the entire first page and be sure not to leave anything blank.

Submit a Claim Electronically

Your Doctor can also submit a claim electronically, using Emedeon (formerly WebMD). This gives a faster turn-around time than submitting a claim by mail. Check with your doctor to see if they are a participant. If they are, show them your ID Card. The Emedeon information is printed on the back of every ID Card.

UnitedHealth Allies does not make payments directly to the providers of medical services. The program member is obligated to pay for all health care services but will receive a discount from those healthcare providers who have contracted with the discount plan organization.

Welcome to the Waive Your School’s Insurance Center!

This page is for those students who wish to opt-out (waive) out of the coverage under their school’s student insurance plan, and have the fee removed from their student account. The school’s student insurance plan can only be waived if the student has coverage under another acceptable insurance plan.

Submitting a waiver, is as easy as 123! The first step to waive out of your school’s student insurance coverage is to verify that you are a “hard waiver” student.

2013 – 2014 School Year

IMPORTANT INFORMATION: Open Enrollment Periods for all eligible Students and dependents: If youor your dependent is eligible to purchase the annual coverage and you choose not to enroll before the Annual Enrollment Deadline, you or your dependent will not be eligible to enroll again until the following school year unless you experience a “Life Status” change during the year.

A hard waiver student for your school is:

All full-time undergraduate students taking 12 credit hours or more, all full-time graduate students taking 9 credit hours or more, and any graduate student designated as full time by their department, all international students regardless of credit hours (including English as a Second Language, American Studies Program, and visiting faculty/scholars) and those working as an intern, are considered a hard waiver student. These students are required to have health insurance to attend Fairleigh Dickinson University. You must complete an on-line waiver form or complete an on-line enrollment form by:

  • Annual Waiver Deadline: 10/15/13
  • Spring/Summer Waiver Deadline: 2/28/14 (new incoming spring students only)
  • Summer 1 Waiver Deadline: TBD (new incoming summer students only)
  • Summer 2 Waiver Deadline: TBD (new incoming summer students only)

WAIVE NOW

For those students who do not submit an approved waiver form before the deadline, you will be automatically charged and officially enrolled in the Student Health Insurance Plan. Once enrolled, there are no refunds or cancelations. (Please note, automatic enrollment will occur AFTER the waiver deadline.)

After you verify that you are a hard waiver student and have other insurance, here’s what you will need:

  • Your current health insurance plan ID card;
  • Your health insurance brochure or plan description (or you might want Mom & Dad’s phone number handy to help answer some of the waiver questions).

PLEASE NOTE:  Once you have completed the waiver form, an email will automatically be sent to the email address you provided, notifying you of the waiver request decision. If you receive a:

  • Pending e-mail – your insurance information is being verified and you should receive a second e-mail within a few days.
  • Approval e-mail – your waiver was approved. You should print this e-mail and save it with your records.
  •  Denial e-mail – your waiver was denied. If you would like us to look this over or if you answered a question incorrectly,

When calling the UnitedHealthcare Global Operations Center, please be prepared to provide:

PLEASE NOTE: UnitedHealthcare Global is not travel or medical insurance but a service provider for emergency medical assistance services. All medical costs incurred should be submitted to your health plan and are subject to the policy limits of your health coverage. All assistance services must be arranged and provided by UnitedHealthcare Global.

Access your ID now by visiting MyAccount. From your MyAccount, you can print and download your ID card.

Permanent ID Card

In keeping with our Go Green initiative, permanent ID cards will be available by request only. Should you wish to obtain a permanent ID card, you can request one to be shipped to you by accessing your MyAccount. Permanent ID cards will be shipped  24 – 48 hours of your request.
